Telehealth Informed Consent
Effective Date: January 1, 2026
This Telehealth Informed Consent ("Consent") explains the telehealth services offered by Regener8MD and the medical practice of Anthony L. Capasso, M.D. ("Regener8MD," "we," "our," or "us"), the benefits and risks of telehealth, and your rights and responsibilities. Please read it carefully. You will be asked to sign or electronically accept this Consent during intake, before your first telehealth visit. It remains in effect for all telehealth services you receive from us until you withdraw it in writing.
1. Definitions
"Telehealth" means the delivery of health care services, including assessment, consultation, treatment planning, education, care management, and prescribing where permitted, using secure, real-time two-way audio and video, secure messaging, and the secure transfer of medical information such as lab results, images, and wearable data, between you and a Regener8MD clinician who is not physically with you. "Clinician" means Dr. Capasso or another licensed health care professional working under Regener8MD.
2. Services Covered by This Consent
This Consent applies to telehealth services provided to members of Clinical Membership and Physician Concierge, including physician reviews of your laboratory, epigenetic, mitochondrial, and microbiome results; protocol and treatment planning; nutritional consults; follow-up visits; and secure messaging with your care team. The free Regener8MD app, the Baseline Kit, and the Longevity Kit are wellness and education tools, not medical care, and are governed by our App Terms of Use.
3. How Telehealth Visits Work
- Visits take place through a secure, HIPAA-compliant video platform or, if video is not possible, by telephone.
- Your clinician may review your health history, current medications and supplements, lab results, and data from the Regener8MD app and connected wearables before and during the visit.
- At the start of each visit, we will verify your identity and confirm your physical location and a phone number where we can reach you if the connection is lost.
- Your clinician will document each visit in your medical record, the same as an in-person visit.
4. Expected Benefits
- Access to Dr. Capasso and his team without travel, from wherever you are.
- Faster follow-up and more frequent review of your results and progress.
- Continuity of care between in-person visits.
- The ability to include your own data, such as wearable and app data, in your care.
5. Possible Risks and Limitations
As with any medical service, telehealth has risks. These include, but are not limited to:
- Your clinician cannot perform a hands-on physical examination, and information shared by video or photo may not be enough to make an appropriate medical decision. Your clinician may then require an in-person visit, additional testing, or a referral.
- Technical problems, such as a poor internet connection, equipment failure, or low image resolution, may delay or interrupt a visit or affect the quality of information shared.
- Despite reasonable security measures, electronic communications carry a risk of unauthorized access or interception.
- Delays in evaluation or treatment may occur because of technical failures or the need for in-person follow-up.
- In rare cases, a lack of access to complete medical records may result in adverse drug interactions, allergic reactions, or other judgment errors. You can reduce this risk by giving us complete and accurate information.
6. Your Location and Clinician Licensure
Regener8MD serves clients around the world through the Regener8MD app. Physician telehealth visits are provided only where the law allows: by a clinician licensed in the jurisdiction where you are physically located at the time of the visit, or as otherwise permitted by applicable law. You agree to tell us your true physical location at the start of every visit. If a visit cannot lawfully take place by telehealth from your location, we will tell you and help you arrange an in-person visit at our Jacksonville Beach or Jacksonville, Florida office.
7. Prescriptions
Your clinician decides whether a prescription is clinically appropriate and will not prescribe solely because you request it. Peptides, hormones, and other medications are prescribed only when your results and history support them. Federal and Florida law limit prescribing of controlled substances by telehealth; some prescriptions, including testosterone, require an in-person visit. Medications are filled by licensed pharmacies, which may be located outside Florida and may not ship to every location.
8. Emergencies
Telehealth is not for emergencies. If you are experiencing a medical emergency, call 911 in the United States, your local emergency number outside the United States, or go to the nearest emergency room. If a clinician believes you need emergency care during a visit, you agree that we may contact emergency services at the location you gave us and your designated emergency contact.
9. If the Connection Is Lost
If a visit is interrupted, your clinician will try to reconnect by video and then call the phone number you gave at the start of the visit. If the visit cannot be completed, we will reschedule it at no additional charge.
10. Privacy, Security, and Recording
- Your health information is protected under federal and state law and our HIPAA Notice of Privacy Practices. App and wearable data are also covered by our Consumer Health & Genetic Data Notice.
- Visits are not recorded by audio, video, or screenshot without the written consent of everyone on the visit. You agree not to record visits without your clinician's written consent.
- You will be told who is present on our side of the visit. You may have a family member or other person join you; by doing so, you consent to that person hearing your health information.
- Please join from a private location, on a secure network, using your own device.
11. Your Responsibilities
- Provide complete and accurate information about your health, medications, supplements, and location.
- Follow your clinician's instructions, including recommendations for in-person care, testing, or referral.
- Keep your Regener8MD account credentials secure.
- Do not use secure messaging for urgent needs; messages are reviewed during business hours.
12. Coordination With Your Other Clinicians
With your permission, we may share your visit summary and protocol with your primary care physician or other treating clinicians. We encourage you to keep a primary care physician for your general medical needs.
13. Fees
Telehealth visits included in Clinical Membership or Physician Concierge carry no additional fee. Other services are billed as described in your membership agreement. Regener8MD is a cash-pay practice and does not bill insurance; memberships are not health insurance.
14. Your Rights
- You may decline telehealth and request an in-person visit at any time, without affecting your right to future care.
- You may withdraw this Consent at any time by notifying us in writing. Withdrawal does not affect services already provided.
- You have the same rights to access and receive copies of your medical records from telehealth visits as from in-person visits.
- You may ask questions about telehealth at any time before or during a visit.
